MENTAL HEALTH PRACTITIONER - Ramsey County
Status: Full-Time
Program: Employment Service Ramsey County MFIP
Location: 1612 Como Ave., St Paul, MN (Como Crossing) or 1910 County Road B, Roseville, MN (Fairview Community Center)
POSITION SUMMARY: The Mental Health Practitioner will provide Adult Rehabilitative Mental Health Services (ARMHS) to qualified MFIP participants who experience multiple barriers to employment including mental illness through individualized and group instruction training.
ESSENTIAL RESPONSIBILITIES:
1. Referral screening to determine participant eligibility with respect to program admission criteria and policies and procedures.
2. Completion of intake paperwork, including functional, employment (Employability Measure) and cultural assessment on each newly admitted FSS participants.
3. Provide sufficient “billable” ARMHS rehabilitative services per contract agreement (average of $2,500 per month).
4. Develop and monitor Individualized Case Plans, with full participant input and consent.
5. Attend and participate in all required meetings.
6. Provide individualized ARMHS services, specifically skills development training through regular community and 1:1 instruction to promote maintenance of stable functioning and self-sufficiency.
7. Conduct FSS sanction outreach for ARMHS participants at risk of sanction.
8. Facilitate release and exchange of information with RC MFIP staff and other community providers as needed.
9. Assist participants in accessing and utilizing resources in their communities.
10. Plan and co-facilitate ARMHS/FSS groups.
11. Maintain participant data and prepare reports as needed.
12. Establish and maintain necessary professional relationships with other community resources, agencies, and service providers.
Qualifications
1. Minimum Bachelor of Arts degree in one of the behavioral sciences or related fields from an accredited college or university and 2000 hours of supervised experience in delivery of clinical services to individuals with serious and persistent mental illness is required or a Master’s degree in Human Services.
2. Meets MH Professional, or MH Practitioner status as defined by DHS, Adult Rehabilitative Mental Health Service Providers criteria/standards.
3. Experience working with multi-cultural population is preferred.
SALARY RANGE: Low to Mid-30s DOQ
